Description:

Synthetic glucocorticosteroid with anti-inflammatory, immunosuppressive, and anti-allergic effects. Used for conditions requiring systemic hormonal therapy.

Indications:

– Endocrine disorders: adrenal insufficiency, congenital adrenal hyperplasia.

– Rheumatic diseases: rheumatoid arthritis, juvenile arthritis.

– Autoimmune diseases: systemic lupus erythematosus, dermatomyositis.

– Allergic conditions: severe rhinitis, bronchial asthma, serum sickness.

– Skin diseases: pemphigus vulgaris.

– Hematological disorders: idiopathic thrombocytopenic purpura.

– Neoplasms: leukemias, lymphomas.

– GI diseases: ulcerative colitis, Crohn’s disease.

– Other: sarcoidosis, tuberculous meningitis (with anti-TB therapy).

Dosage and Administration:

– Initial dose depends on the disease (see table in leaflet).

– Maintenance dose: minimal effective, taken once in the morning or every other day.

– Withdrawal: gradual, under medical supervision.

– Children and elderly: use with caution, minimal effective dose.

Examples:

Rheumatoid arthritis: 4–16 mg/day.

Systemic lupus: 20–100 mg/day.

Bronchial asthma: up to 64 mg/day.

Contraindications:

– Systemic fungal infections.

– Hypersensitivity to methylprednisolone or excipients.

– Live vaccination during immunosuppression.

– Severe infections without specific therapy.

Precautions:

– Infections: Increased susceptibility to bacterial, viral, fungal infections.

– Endocrine effects: Adrenal suppression, Cushing’s syndrome.

– GI: Ulcers, perforation, pancreatitis.

– Cardiovascular: Hypertension, thrombosis.

– Nervous system: Psychiatric reactions (depression, mania).

– Ophthalmology: Cataracts, glaucoma.

– Metabolism: Hyperglycemia, hypokalemia.

– Bones: Osteoporosis (especially in postmenopausal women).

Adverse Effects:

Common (>1%):

Sleep disturbances, weight gain, edema.

Dyspepsia, hyperglycemia.

Rare (<0.1%):

Thrombosis, osteoporosis, psychiatric reactions.

Cataracts, glaucoma.

Long-term use: Adrenal insufficiency.

Pregnancy and Lactation:

Pregnancy: Only if strictly indicated (risk of fetal growth retardation).

Lactation: Excreted in breast milk (use with caution).

Storage:

Store below 30°C in a dry place.
