Description

Mavilor is a selective beta1-adrenergic receptor blocker with vasodilating properties, used to lower blood pressure and treat chronic heart failure. It decreases heart rate, reduces vascular resistance, and improves cardiac function, particularly in elderly patients.

Indications for Use

Treatment of essential hypertension.

Treatment of stable mild to moderate chronic heart failure in elderly patients (≥70 years) as an add-on to standard therapies.

Dosage and Method of Administration

-Hypertension:

Adults: 5 mg once daily at the same time each day. Effect appears within 1–2 weeks, optimal after 4 weeks.

-Renal impairment and elderly: start with 2.5 mg daily.

-Hepatic impairment: contraindicated.

-Paediatric population: not recommended under 18 years of age.

-Chronic Heart Failure (CHF):

-Start with 1.25 mg once daily, titrated every 1–2 weeks based on tolerance to a max of 10 mg/day.

-Only under supervision of experienced physician.

-In case of deterioration – reduce dose or discontinue.

-Long-term treatment. Discontinuation must be gradual.

Method of administration:

For oral use. Swallow with sufficient fluid (e.g., a glass of water). Can be taken with or without food.

Contraindications

Hypersensitivity to nebivolol or any excipients.

Liver insufficiency or impairment.

Acute heart failure, cardiogenic shock.

Bradycardia (<60 bpm), hypotension (<90 mmHg).

Sick sinus syndrome, AV block (2nd/3rd degree), untreated pheochromocytoma, metabolic acidosis.

Asthma or bronchospasm history.

Precautions

-Do not discontinue abruptly.

Use with caution in:

-peripheral circulation disorders,

-diabetes mellitus (may mask hypoglycemia),

-hyperthyroidism (may mask tachycardia),

-COPD,

-history of psoriasis.

-May increase sensitivity to allergens.

Not recommended with:

-verapamil, diltiazem,

-class I antiarrhythmics,

-centrally acting antihypertensives.

-Contains lactose – not for patients with galactose intolerance, Lapp-lactase deficiency or glucose-galactose malabsorption.

-Sodium content: essentially sodium-free (<23 mg per tablet).

Side Effects

Common: headache, dizziness, fatigue, bradycardia, dyspnea, constipation, nausea.

Rare: allergic reactions, angioedema, nightmares, depression, rash, worsening of CHF.

Pregnancy and Breastfeeding

Pregnancy: may impair placental perfusion; associated with intrauterine growth retardation, death. Use only if clearly necessary and monitor foetus.

Breastfeeding: not recommended due to unknown excretion into human milk.

Storage Conditions

Store below 30°C, in original packaging, protected from light and moisture. Keep out of reach of children.
